- Transportation into our care (within a 45-mile radius)
- Standard urn
- Alternative container
- All necessary authorizations and cremation fees

- Please submit a photo and obituary of your loved one by email, text, or hand delivery.
Optional Items:
- Remains (Bodies) between 250 - 350 lbs is an additional $200
- Remains (Bodies) between 350 - 450 lbs is an additional $350
- Remains (Bodies) between 450 lbs and above is an additional $500
- Cremains Shipping and Handling is an additional $100

Basic Services of Funeral Director and Staff, transportation into our care (within a 45-mile radius), transportation to crematory, alternative container, all necessary authorizations, and cremation fees; 30-minute visitation followed by one-hour memorial service (M-F) in the funeral home chapel before 4 p.m., guest register book; one (1ct) certified death certificate; fifty (50ct) memorial cards or memorial programs, standard urn, includes weight up to 250 lbs.
Additional:
$500 upcharge for Saturday, Sunday, and Monday through Friday after 4 p.m. (evening) memorial services.

$500 upcharge for Saturday, Sunday, and Monday through Friday after 4 p.m. (evening) services.

Guidelines To Follow:
- Notify the Hospital, Hospice, or Nursing Facility to call Funeral Home at 937-521-4800 once your loved one has passed. (This information can be provided to Facility Institution before or after.) We do not need any paperwork in order to pick up your loved one, just verbal authorization from the Next of Kin or Person in Charge.
- Before or after your loved one passes, please print the forms below, fill them out by hand and fax, email, or deliver by postal service to 1307 E High St, Springfield, Ohio 45505. (You may also take a picture of the forms with your smartphone.)
- If you do not have access to these forms, you may make an appointment with our Licensed Funeral Directors to finalize arrangements.
- Payment may be made at the Funeral Home in the form of cash, cashier's check, money order, or any major credit card.
